Patient Responsibilities

Appointment Policies

Emergency Care

We provide emergency dental care during business hours and have after-hours coverage for true dental emergencies. Emergency fees may apply for after-hours care.

Payment Terms and Financial Policies

We accept cash, checks, and major credit cards. We also work with CareCredit and offer payment plans for qualifying patients.

Insurance Claims

We file insurance claims as a courtesy to our patients. However, your insurance policy is a contract between you and your insurance company. You are responsible for payment regardless of insurance coverage.

Collection Policies

Accounts not paid within 90 days may be sent to a collection agency. You will be responsible for all collection costs, including reasonable attorney fees.

Patient Records

We maintain patient records in accordance with professional standards and legal requirements. Records are typically retained for a minimum of seven years after the last treatment date. You have the right to access your records and request copies upon written request.

Referrals

When necessary, we may refer you to specialists or other healthcare providers. You are responsible for scheduling and payment of services provided by other practitioners, even if referred by our office.

Termination of Services

We reserve the right to terminate the doctor-patient relationship and discontinue services for reasons including but not limited to:

In case of termination, we will provide reasonable notice and assist in transferring records to another provider.
